Severus/Poppy is also a favorite, along with Severus/Regulus, Severus/Tonks (love!) and Severus/Luna. I have a secret fondness for Severus/Sirius hatesex, and I'm getting interested in Severus/Voldemort. Beyond that:

Severus/Lucius/Hermione (if this counts as rare?)
Minerva/Severus
Minerva/Poppy
